The Property Assessment Division has posted a revised form on the website:

Form 457 Application for Exemption from Motor Vehicle Taxes by Qualified Nonprofit Organizations

Minor changes were made to the text and instructions to make the form consistent with the Department's standards.

The revised Form 457 is effective immediately; however, county treasurers may accept applications filed for August and September on the old Form 457.

Please discard any supply of the old Form 457 and use the current form on the web.

Applicants may use the fill-in version of the current Form 457 available on the website.
