The Property Assessment Division has posted revised forms to the website as follows:

Claim for Personal Property Exemption, Form 775P and Schedules I, II, and III; Minor changes were made to the forms and instructions to make them consistent with the Department's standards.

Claim for Personal Property Exemption, Form 312P and Schedules I, II, and III; The form number changed from 5725X to 312P. Minor changes were made to the forms and instructions to make them consistent with the Department's standards.

Only the revised versions of these forms will be accepted by the Department for the filing period ending May 1, 2013.
